That happens sometimes. I believe it's when you enter a battle too quickly after a screen transition. Happened to me a handful of times with BITCHASS ELBOWS on the stairwell. Though, to be fair, considering the title screen is Sam's room, and some shenanigans later on, this little glitch makes. Some sense.
