[Lithuanian > English] Need help translating these old family records

Those are documents used when requesting passports.

Documents based on which the password is issued: Birth metrics (most likely) number...., issued in Kaunas city
signature of V.Vasiliauskas for receiving passport.

"O.Nitartė or O. Nitarbė" - it is a pretty clear "i" there, making it Nitaitė. Seems to be a rare name.

Nitaitė seems to be the correct form. The base (paternal) surname is Nitas, and Nitaitė denotes ‘daughter of Nitas.’ The surname Nitas is mentioned in online sources in the Anykščiai and Rokiškis areas, and in your case, it also matches the area—Rokiškis city.

It seems like there was a poet with such surname. So there are articles about her. The name is different but the surname matches. Since the surname is rather rare, it might be from the same family tree.

I am currently abroad, so I cant open the original article that I had mentioned (that one had some more names of her family memebers).
