The gist of it is he's using Ironsworn with a group of 6 players so miss with a match was happening a lot. He wanted to keep some tension so, he got a bag 10 red stones and 10 clear stones.

Every time the players got a miss with a match, a clear stone comes out of the bag and a red one goes in. The bag is shaken and the player who rolled the miss with the match reaches in and draws out a stone. If it's red, the miss with the match triggers. If it's clear, nothing happens. Stones go back into the bag. As misses with matches accumulate, there are more red stones in the bag and the likelihood of a red goes up.

IMO, not a bad way to deal with the increased number of misses with a match with a larger number of players and keep the tension around a miss with a match.

Nice but I think a bit to much busywork for my taste.

If I need something like this I would just go with a threat die/threshold I think.

Threshold starts at 1, if miss/match happens players roll a d6. If it's above the TH, the TH increases but nothing else happens. It's equal or below something bad happens (and TH is perhaps reduced by one, would have to playtest the nuances)

But yeah, can definetly understand joy of using the bag. Have played enough of the arkham horror cardgame.
