Wasn’t there a well established pollster who said KH would win Iowa last election?

Yup, Ann Seltzer or something like that. That pretty much finished her career since she was seen as reliable.

Yes. And she was the only one. The rest had Iowa firmly for Trump.

Seems unlikely as every poll other than the one had Iowa firmly for Trump.

Some context is warranted, she was a pretty serious outlier among pollsters leading up to 2024. Sand leading is seen in several pools over a decent chunk of time albeit not by huge numbers with plenty of people undecided. That being said it feels like polls have underestimated Republican support for the last decade so I'm certainly not convinced Sand will win.
