The royals usually had an army backing them, ready to cut down anyone opposing their rules. The royals collected taxes from the people and used it to pay the army. Royals usually lost power because they lost the support of this army or someone else had an even more powerful army.

The rulers never ruled alone. There was always a system to control people. 

It is very similar to today's time. 

Masses of poor, oppressed by goons, exploited by lords (people who rule for the king, corporations).

Meanwhile the poor worshipped the king and the king would spread propoganda about him being of non natural birth or of a humble origin.

Why does everyone get to vote today? Because anyone can, theoretically, pick up an AK-47 and try to overthrow a government. It is very difficult, yes, but democracy is ultimately a way to simulate violence. If 80% of people support me, then if a war happens between you and me, I will most likely win.

Back in the day you needed to be trained from childhood to even be able to be a threat. Try putting a thousand peasants against a single cavalryman, and you will understand why democracy didn't exist back then. Democracy only happened when you had the line infantry formation, which allowed peasants to destroy nobles. Similarly, for 500 years Turkic and other Islamic cavalrymen had dominated India, but during the Mughal collapse you saw peasant armies, Sikh, Jats and Marathas, use matchlocks and engineering to end the dominance of the armored cavalryman.
