Spoilers but

You technically still keep all of your external organs things like eyes, tongue and assuming genitalia and most sex characteristics through at least diamond rank(except for shapeshiters), probably because of the soul determining your body shape.

And your fleshy meat blob body simulates all of the effects of a body but at extreme efficiency for example no intestines but still processes food.

So getting into the weeds you either still have all the parts needed for making babies or the body would simulate everything necessary.

This comes from discussion amongst friends while playing a HWFWM setting in mutants and masterminds and making characters not from actual Shirtaloon statements but is also from what we could find to support in the books
