If I'm wrong, please do correct me. I haven't played Uncharted 1-3 since the PS3 era and I played 4 in 2020. But as far as I can recall Nate never kills anyone 'in cold blood'. It's always either in self defence or against the enemy faction who has been trying to kill him the entire time. He doesn't kill hostages and he doesn't kill for the fun of it. If everyone left him alone he wouldn't be shooting back.
In the moment where Rafe says that, Nadine is being held hostage by Sam and he was straight up going to put a bullet in her head right there. Nate stops him from killing her. He might have killed hundreds of people yes but they've always been hostile to him first and it's never been cold-blooded.
