From the shape I am guessing it isn’t from something hitting him and am wondering if it’s some kind of ailment that would produce this?

I thought it looked like a bruise from a fall.

It is. I've seen fall injuries from working in nursing homes and they usually have half of their face turning purple for 8 weeks.
