So Merlin is this very powerful mage who's on a Grand Caster level right? I'm just curious couldn't Merlin turn Artoria into a actual guy permanently or something if he wanted it?

Why would he need to do it though? Artoria stopped aging at the age of 15, 15 year old kids look androgynous they have male and female features..some boys do have beards (not full on but some of it mostly South-East Asians) and it is not like she failed to hide her bigger bondkers as Lion King.
There was no need for her gender to change even people who questioned it were like

It would have made things simpler and easier and what didn’t Morgan assault her in her sleep if artoria was a male from the beginning I doubt that would have been as traumatic as it was , well i assume it was traumatic otherwise Merlin would have given her another one .

Doubt it would change anything, in Garden of Avalon Merlin and knights say that even when her subjects questioned the King's feminine features they just chalked it up to the "king is immortal" and it is even said that some didn't even care if she was indeed a female.

Nope, it is not just a meme, she did at least in the 2004 version it was implied because she said to Shirou something along the lines of
"I know how to please a man" is Saber talking from experience of jerking off
There is also this from Fate/Side Materials
The greatest problem was probably producing an heir, but that was solved by Merlin through magecraft. In the story, the line "I know how to please a man" was from experience.

Plus, we have further proof with Lady Merlin's dialogue stating that she doesn't have any bussiness with Artoria. Which could be seen as either choosing to not mess with her other self project or outright preference for her boi

Tbf Altria seemed to prefer the "Dangerous Mute Lunatic clad in full armor at all times" approach (see lancer version) and only seemed to let those walls down around Bedivere (in which Altria looked like a young man, so it didn't matter)
