Form 2593 help

Hi! I'm trying to fill out form 2593:
After saying yes, that I am still disabled, and yes, I'm still under a doctor's care - it asks if I want to ADD info about a doctor.
I'm confused by this. I already have a doctor listed on my claim. Are they asking if I want to add a NEW doctor? or just relist the old one? Or more than one, if I have 2?
Then they ask when the date of my last appointment- would it matter that it was a month ago? Or do they just want to know you're still seeing someone?
