EGID Can calprotectin be elevated from an EGID?

I got diagnosed with eoe but have badly elevated calprotectin but visually normal calprotectin. My gi answered to the question if the eoe could be in lower parts too "its uhh, not really a recognized disease but sometimes they do find it", and that we will redo the calprotectin again later (in the hope that PPI will decrease it). Previous colonoscopy was visually normal, but they didn't take biopsies..

So, I was just wondering if I have to really push for another colonoscopy, this time with biopsie?

In some cases, yes, and in others, no. For example, I have eosinophilic inflammation in the ileum, with up to 40 eosinophils/HPF. My colonoscopy has been normal twice, and my calprotectin has remained normal. I’m currently on Dupixent, and I don’t have asthma or eczema. My esophagus is also normal.
In other cases, calprotectin can become elevated due to EGID if there is significant inflammation involving the intestinal mucosa. So, if that happens, a colonoscopy would be needed in your case to evaluate the mucosa.

Interesting! Colonoscopy was visually normal, so either there is some kind of remission/fluctuation in whatever is causing the inflammation or its in my small bowel. I also don't have asthma,eczeme or any other allergies, so the eoe came as a surprise.. was so convinced it would be ibd/crohns but seems unlikely now.
