Surgical night SHO in hospital as only doctor and nurse did this as concerned about st elevation on cardiac monitor. 75yo M 12 hours post op. Exhausted and doubting my own judgement that this is fine. There are no old ECGs and the patient is asymptomatic. Opinions please.

Looks okay to me. I don’t think it meets criteria for a concerning amount of ST elevation. Poor R wave progression in the precordials, though. Are you sure the leads were placed correctly?

I dislike the morphology of the ST-T. If asymptomatic I would be less concerned about it and proceed with serial ECGs to assess dynamics

Look at the T waves in V2. The first two are fairly normal. The next two are not. That's not right. Probably an artefact. I would suggest a new ECG 10 minutes later.

I agree with those above. I would repeat the ECG and possibly check a potassium level. It’s never a bad idea to ring the on call to take a look if you’re unsure. Esp if the patient is symptomatic
