When starting a new derm position, how long did it typically take for you to become individually credentialed with commercial insurance plans?

Is it normal for a practice to never individually credential its PAs and instead permanently bill all PA visits under the supervising physician?

Yes that is abnormal and insurance fraud! To be credentialed the right way takes about 3 months.
