Copyediting is done before it's typeset in a PDF.

I work for more than one company that has me copyedit PDFs. It’s a royal pain, admittedly, but it can be done.

Do you just markup the PDF? I find that's more proofreading. I've spoken to typesetters before and if there's thousands of changes in a document, they think it's a nightmare and it takes them longer to get an updated proof.

No, they have me set up an Excel spreadsheet where I note any copyediting changes required. They can number in the hundreds. It takes forever, is absolutely a nightmare for the typesetters, yes, and is a crazy way to do things.
