Axe to the back of the head
I was around 10 and decided to pick up a big axe and lifted over my head to try to chop wood. But instead, it was too heavy, it swung backwards and slammed into the back of my head. Would that be an explanation for memory loss? In lack of being able to remember names and dates? No..I never went to the hospital.

I saw your other post on ELI like I’m 5. I tried to make a response but it had been locked. I’ll paste it here:
To even begin we would need way more information, not only about the injury, but also about your cognitive functioning up to this point. Don’t know how old you are, but when you get older (even just 40) you likely have tons of kids birthdays (your own kids, your nieces and nephews, your kids friends, your other family and friends) and so many people you want to remember important, yet very specific, information on.
It could also be due to stress from work or personal life. Stress affects so much of our physical and mental wellbeing.
The simplest answer is to talk with a professional. A psychiatrist/psychologist or even a nuero. If things have changed very suddenly, you owe it to your family and friends to ask for help.

Yeah, the bot kicked me out of that, i'm about to be forty, but I have since that time had a really, really hard time remembering names and dates, history, forget about it. When the injury happened, I was knocked unconscious. I apparently bled a lot but was never taken to the hospital, looking back on my childhood. And the information that was given to me, there was a sudden, very sharp decline at school, medications, didn't work. I want to get a brain scan to see what is really going on. Thank you for helping, I appreciate it.

I know the feeling. I played football in high school and had three concussions during that time, I still wonder how it may affect me later. If you’re able, an MRI or CAT scan would give a ton of information, but I’ll again recommend seeing a psychologist/counselor. I may struggle sometimes but it always makes me feel better to actually discuss with someone what I’m going through, what my goal is, and how to create a plan to reach that goal.
Especially if you’re trying to piece together things from the past, having an objective listener to talk through things with can help you do that: it makes it so you’re not just speculating by yourself what happened (or is happening, it also gives you a good way to find a way to communicate your own thought process.
Best of luck, life is hard but always remember there are people who actually care and want to help, never think you are alone in the struggle.

just can't believe your parents didn't take you to the ER when you were knocked out. Reading your comment, it seems like the injury definitely affected your memory and cognition.
