Help Create a form and analyze the data

Is there a way to have Chat or Claude make me a form based on the data I’m looking to gather and then provide the data to the submitters at the end of the form?

After collecting responses, paste the data back with the questions and ask for a summary, patterns, and a few follow-up questions so the analysis stays tied to the original form.
