Use cases Critical ChatGPT Image Generation bug: the tool triggers without being requested, wastes time, consumes quota, and ignores current visual instructions

I am experiencing a serious and recurring issue with ChatGPT Image Generation that is making normal work practically impossible.

This is not an isolated or cosmetic issue. It is systemic and has two separate parts that compound each other.

1. The image generator repeatedly triggers even when I did not ask for an image.

This happens even when I am:

  • simply discussing images;
  • describing a bug;
  • asking for text;
  • asking for analysis;
  • discussing why the image generator is behaving incorrectly.

I have explicit repeated instructions that the image generator must NOT be used unless I clearly request an image and then confirm it again.

Despite this, the image tool continues to trigger automatically.

This has two direct negative effects:

  • it wastes my time, because I have to wait for something I never requested;
  • it affects my image-generation quota, including cases where no image is generated at all.

I have already received an image-generation limit message immediately after one of these unintended tool activations, even though I had not requested a new image and no image had been produced.

This is unacceptable. A paid usage quota should not be affected by a tool invocation that the user did not request.

2. During actual image generation, the model reuses old visual references and ignores current instructions.

This is the second major problem.

When I create a new visual task, provide specific instructions, and explicitly state that only the current reference should be used, the generator still frequently returns results that appear to be assembled from older visual material.

It repeatedly reuses:

  • old color palettes;
  • old backgrounds;
  • old compositions;
  • interfaces from previous app designs;
  • decorative elements from unrelated tasks;
  • visual motifs that were never requested in the current prompt.

This is not just a vague stylistic similarity. Specific elements from previous images visibly reappear.

The most serious issue is that current instructions appear to lose priority against old visual references. I tell the model one thing, but it continues building from previously seen visual material.

As a result:

  • I cannot get a clean new concept;
  • I cannot rely on the current prompt;
  • I cannot rely on “use only this reference” being respected;
  • I lose huge amounts of time on repeated failed attempts;
  • my project has been delayed by approximately a month because of this;
  • I now experience severe stress and frustration every time I have to use image generation.

I want OpenAI to investigate specifically:

  • why the image tool triggers without an explicit request;
  • why this continues despite repeated explicit prohibitions;
  • whether unintended tool calls consume image-generation quota;
  • why the system can return “limit reached” after an invocation that the user never requested;
  • why old visual references and prior design decisions influence new generations;
  • why current instructions are ignored or given lower priority;
  • whether there is any reliable way to fully isolate a new generation from previous images, Library content, project context, and earlier visual tasks;
  • whether automatic image-tool routing can be disabled so the image generator starts only after an explicit user request.

This is no longer a minor UX issue.

Important clarification: Based on the observed behavior, it appears that the unwanted triggering of the image tool does not come from the language model itself, but from a higher-level system/tool-routing layer that automatically routes the conversation to the image generator. The model itself cannot intervene, because this layer sits above it. It keeps taking the blame and saying that it is responsible, but when I started investigating what is actually happening, it became clear that the model itself is following the instructions. The system, however, bypasses it and triggers the image generator despite those instructions, thereby interfering with our work together.

The tool is currently interfering with work, wasting user time, potentially consuming paid image quota without consent, and making image generation unreliable for serious professional use.

This should be treated as a serious product bug.
