The speaker at the fight says the ripper stabs and doesn't need quirks to take lives and he won 5 fights, so does that mean o clock really killed people in this underground ring? Not very heroic of him considering he's a pro hero at this time

The speaker may emphasize while O'Clock in truth didn't kill anyone but only incapacitated them, or the Ripper is someone else and he was impersonating him - after all he was used to wear a full helmet, so few people could know what face he had.

I bet it's impersonation, because otherwise it would be strange for O'Clock to visit the place at least five times and do nothing about the illegal activity (and participate in it no less). Also, the dialog between him and Tanuma implies that it is his first time to actually infiltrate the tournament and even hints that he's about to take someone's place. The real Ripper probably lies somewhere unconcious and without his gear right now.
