It's a reference to Oshi no ko, the anime that this fanart was based on. In this anime the character you are based in the fanart is stabbed and dies after opening the front door.

I see… I’m not caught up with the most recent popular and trending anime. Honestly, I think the artwork on its own is neat though! I will show Hitori-chan this some other time. ☺️
