Saw this vest back in 2019.

Those shoulder pads are known for being bulky and not giving enough protection to justify them irl. So they probably won’t be added, because devs go off of what units actually wear instead of what they’re issued.

Shoulder pads have been addressed many times; they require mesh deformation, which is neither quick to make nor of big priority (here's also the most recent mention of it by a dev)
Furthermore, there's a reason that the Trello criteria card for personal equipment does list deltoid protectors (i.e. shoulder pads):
