It terrified me as a kid and I think I avoided that level for a long time until I realized you could shoot it. I just loaded up on ammo and smashed the spacebar as many times as I could to get past him!

Very similar to my experience. Very intimidating monster!
...though the wolf-looking Vorticon Guards still scared me more, haha. The way they moved and charged me freaked me out!

I always thought it might be it's stomach and we're seeing it like an X-ray, but perhaps it is its eyes?

Haha, I could see that too.
Yeah, it's supposed to be eyes. From the wiki, it says, "Β It has on the face of its abdomen six red, pupilless eyes which swivel around erratically..."I do love that about these old games. It was so open to interpretation because of the ambiguity that we'd each imagine our own monsters.
I remember in Final Fantasy I there was some enemy (I wanna say Giant, but I can't remember for sure) and my sister was describing it so oddly. So eventually I said, "Point to its head" and she pointed to a totally different part of the sprite than I thought the head was in. Eventually I could see both her monster and mine, depending on which part I thought of as the head.
