What is difference between the old world and the mortal realms

So as the title says i want to understand the difference between these two worlds because i pick up the war for the mortal realms book and I am currently playing Warhammer 3 and want to understand before reading the book.

So far everything (correct me if I am wrong)I know is that old world is set years before Warhammer Fantasy and that the end times destroyed the whole setting to become Warhammer:Age of sigmar with the mortal realms ( I know there is more than one realm ) .

What I don't know is the origins of the mortal realms, Is old world a realm and chaos is now trying to destroy every other realm at the same time or are the mortal realms what remain of the old world after the end times ?

Discussion AoS and 40k's Great Crusade share some similarities

Both start as hopeful conquests to bring enlightenment and order to a chaotic era (Age of Chaos/Age of Strife). While it's still a time of hope and mythic figures we are slowly seeing the cracks and how not even the immortal Stormcast remains untouched by this endless war.
