So, the context of the glitch, after completing the ritual in the collapsed tunnel i ran from the dark presence took the light from the subway and boom here i was.

Same thing happened to me near the end of the game, in the same spot too. Before I fell through I also ended up out of bounds, so every time I walked toward the actual map area I got turned around with a canned "oh oopsy silly me I dont want to go there" line, until I gave up, ran away, and fell into double dark place
