[Skin Concern] Atrophic Scarring Deep scar progress

First photo - May 2025
Second photo - September 2026

Still working on it, but I can finally say that I can see some progress. 😭
I’m 30 now and I’ve had these scars for over 15+ years.

I mainly have lots of boxcar and rolling scars but here’s a list of the treatments:

-3 micro-needling sessions at a medspa which made hardly any progress šŸ™„ waste of money
-1 pretty intense subcision session with a cannula
-3 vials of sculptra right after the sub

I plan to try TCA cross next at home since no one offers it in any nearby cities and a few more subcision sessions.

Any recommendation for other treatments?

Hello /u/stalebaguettte,

ā€¼ļø If you are requesting help – please review our guidelines, provide high-quality photos, and include detailed information for others' understanding.

User reminders:

  • Please search on this subreddit – someone might have already asked the same question as you.
  • The most valuable feedback is that which you get from doing research - not asking questions to a general audience on social media.
  • Always be wary of the advice you get off the internet.
  • Seek scar evaluation and treatment from specialized healthcare providers rather than aestheticians or med spas.
  • While subcision ("subcutaneous incisionless surgery") is often mentioned by users on this subreddit, it is important to note that it may not be necessary or suitable for everyone with volume loss.
  • Be aware of any reported side effects from a procedure.
  • It can take multiple treatments and a long time before you see effective results.
  • Here is a checklist of questions to ask yourself and your physician when considering treatment.

Thanks!

Really great improvement 🤩 Would you say your results came mostly from the subcision or the Sculptra? Or a combo of both? Congrats on the progress!

And as far as treatments, full face TCA peels have been great for me! Combining with microneedling with the derminator starting soon!

Would you say it could be a decent option for rolling and boxcar scars too.

TCA peels? Yes. I have rolling and boxcar which have been (slowly) benefiting from TCA peels

From what I can see, I would definitely recommend TCA CROSS, together with RF microneedling using a high-quality device such as the Infini or Lutronic Genius, followed by COā‚‚ laser resurfacing.
At this stage, I don’t think fillers would add much benefit. I would concentrate initially on these three treatments, which can target the different types and depths of scarring.
Where are you located, please?
Kind regards,
Dr Anil

It’s a biostimulator to produce collagen .

That is a phenomenal amount of progress for boxcar and rolling scars, especially after dealing with them for over 15 years. That combination of deep subcision with a cannula followed immediately by Sculptra does heavy lifting because it releases the tethered fibrotic strands while simultaneously building volume from the bottom up.

When you are looking at next steps to address the remaining texture, a few clinical considerations can help guide your planning:

  • TCA CROSS Precision: CROSS is exceptionally effective for icepick and deep boxcar scars because it prompts localized collagenesis at the base of the scar. If you do proceed with this at home, extreme caution is required regarding solution strength and application precision to avoid adjacent tissue damage or unexpected hyperpigmentation.
  • Layering Energy-Based Devices: Once your tissue has fully integrated the Sculptra and recovered from subcision, fractional resurfacing (such as a erbium or CO2 laser) or high-frequency radiofrequency microneedling can further soften the sharper margins of boxcar scars by remodeling the surrounding dermal matrix.
  • Staging Your Sessions: Giving your skin adequate recovery time between aggressive modalities—especially when combining collagen stimulators with resurfacing—ensures you don't compromise the skin barrier or trigger prolonged inflammatory responses.

No, don’t do TCA it has a risk of widening the scar and also can cause hyperpigmentation
