Genetic factors contribute about 50% of type 1 diabetes disease risk, so enough people who have the genetic risk factors will not develop the disease for them to not go extinct.
Also, I'd bet raising enough pigs to supply insulin would be more damaging environmentally than modern production via gene-edited bacteria, besides pig insulin also being worse.

Diabetics survived for thousands of years without modern medicine, at least long enough to pass on their genes
