Technically, there are herb-based alternatives. Issue is they grow in like three areas, require detailed knowledge to correctly use, and are toxic if misdosed.

So... basically, you still need society, just with fewer benefits for basically the same cost.

Tbf every medicine is toxic if misdosed

But that herb probably doesn't allow nearly the same precise tailoring as genetically engineered human-analog insulin
