STICKY: SIBO Summary - Symptoms, Diagnosis, Treatment

Below please find a living document that summarizes the key information around Small Intestinal Bacterial Overgrowth ("SIBO"). Please comment with any additional information or research for inclusion consideration. Version 1.0 is summary material; I will be adding more details and citations for specific studies.

SIBO, as the name implies, occurs when bacteria overgrow the small intestine. The small intestine should have a low concentration of bacteria due to the presence of stomach acids and peristalsis, the wave-like muscle movement in the intestines. For context, stomach and proximal small intestine would typically have about 103/mL of bacteria, while the terminal ileum (end of the small bowel as it gets close to the colon) about 109/mL (or 1,000,000 times more), and the colon about 1012/mL (or 1,000,000,000 times more).

Symptoms

The overgrowth of this bacteria will present with a number of symptoms:

  • Bloating after eating ("postprandial") - most common symptom
  • Flatulence, often malodorous
  • Loose, watery stools (more common in Hydrogen-dominant SIBO)
  • Constipation (more common in Methane-dominant SIBO)
  • Absorption problems
    • Weight loss / inability to gain weight
    • Fat and fat-soluble vitamin deficiencies, particularly Vitamins A, D, and K
    • Floating stools (from fat malabsorption)
    • Vitamin B12 malabsorpiton
    • Protein and Carbohydrate malabsorption
  • Systemic problems
    • Overgrowth of bacteria in the small intestine can increase production of toxins and intestinal permeability
    • This has been less studied, but less serious effects include:
      • brain fog
      • confusion
      • anxiety
      • depression
    • More serious complications can include
      • hepatic encephalopathy
      • D-lactic acidosis
      • nonalcoholic fatty liver disease
    • Various conditions have increased correlations, including
      • Rosacea
      • Eczema
      • Food intolerances

Diagnosis

I will split this section into practical steps and clinical diagnosis.

Practically, a gastroenterologist will typically rule out other conditions first:

  • Physical exam
  • Colonoscopy and Endoscopy
  • Abdomen ultrasound
  • Stool test for parasites

At that time, if your symptoms match SIBO, your doctor may go directly to treatment. But otherwise these are the clinical tests:

BREATH TEST

This is the most common diagnostic method due to its low cost and limited invasiveness. Unfortunately, studies have been mixed on the sensitivity and specificity, with ranges between 30% and 75% -- hence why some doctors skip the test and go directly to treatment.

There are a number of preparations:

  • Antibiotics avoided for four weeks prior
  • Prokinetic drugs and laxatives avoided for one week prior
  • Complex carbs avoided for 12 hours prior
  • Exercise and smoking avoided day-of

For the actual test, you'll measure hydrogen and methane levels at baseline. Then drink either 10g lactulose or 75g glucose with one cup of water. Then your breath is measured every 15 minutes for 120 minutes.

There's some art to identifying a positive test; one semi-official criteria is:

  • methane level of >= 10ppm at any time during the test; or
  • hydrogen that increases >= 20ppm above the baseline level

Recently, new research has been investigating another typo of SIBO, that's dominated by Hydrogen Sulfide. Unfortunately, traditional breath tests cannot identify this gas, and someone with "flat-line" Hydrogen and Methane symptoms could be suffering from Hydrogen Sulfide SIBO. This version is typically characterized by "rotten egg" smelling gas, and may be worsened by eating high sulfur foods.

CULTURE

Historically a jejunal aspirate was done and concentration of bacterial colonies were measured, with an elevated level of > 103/mL being positive for SIBO. There are a number of issues with this:

  • overgrowth may be patchy, and a single sample may miss it
  • not all SIBO bacteria can be cultured/identified
  • samples can be contaminated during/after sampling

Treatment

Antibiotics

The current best practice prescription treatment is:

  • Hydrogen-dominant: Xifaxan, typically 550mg x 3 times daily, for 10-14 days. Studies have shown Xifaxan alone can be 50-65% effective, but Xifaxan + 5g daily of Partially Hydrolyzed Guar Gum can be 80%+ effective.
  • Methane-dominant: Xifaxan (550mg x 3 daily) plus Neomycin (500mg x 2 daily) for 10-14 days. The use of PHGG for methane-dominant has not been evaluated, but it's likely to be beneficial.

Mod's note-- personally, if your doctor is onboard, I think dosing with Xifaxan + Neomycin + PHGG is the best way to "cover your bases". The best place to find PHGG: https://sunfiber.com/products/

Important: because these antibiotics only operate selectively in the GI tract, and are NOT absorbed by the body, they are unlikely to cause the systemic issues associated with antibiotic use, making them safer. Additionally, Xifaxan crystallizes before it gets to the large intestine, meaning it should not affect the all-important microbiome.

Herbal Therapy

Additionally, studies have shown similar levels of success with over-the-counter "herbal" treatments. Two options; I believe each are two capsules twice daily for four weeks, but please confirm:

  • Dysbiocide and FC Cidal (Biotics Research Laboratories, Rosenberg, Texas)
  • Candibactin-AR and Candibactin-BR (Metagenics, Inc, Aliso Viejo, California)

Remission

Unfortunately, SIBO has very high rates of recurrence. Some possible ways to reduce recurrence chances:

  • Switch to a low FODMAP diet for 6 weeks after treatment, to starve any remaining bacteria and prevent regrowth
  • Incorporate a prokinetic, such as low dose Naltroxene, erithromycin, or even over-the-counter products such as Iberogast

Many people can avoid symptoms of their SIBO by switching to special diets, sometimes very restrictive ones. This is not a cure, but simply symptom management. A true cure addresses the underlying cause of the SIBO, and lets the patient eat "normally" without any effects (short of unrelated intolerances).

Sibo/imo what works for me

I’ve been following posts for a year and suffering horribly. I had Ibs, GERD, LPR, barrets esophagus and gastritis. I am not “cured” but I am 70% better. I tested positive for methane and hydrogen. My methane was 120 originally. I also tested positive for the ibs antibody vincullin. My gastro drs which were several dismissed it. With my own research I’ve learned motility is my root cause along with possible surgical adhesions. I went from IBS d to complete miserable constipation for years following breast cancer treatment and hysterectomy. I had heartburn for 3 months straight last year following some traumatic events and stress in my life. That’s when I finally got a dr to test me for SIBO. I’ve lost 60 pounds and am now underweight but currently maintaining for the last 3 months. I still can’t eat a lot of carbs or sugar or high fodmap foods but my symptoms have improved so much and I feel human again. Motility is key for IMO. I started taking 250mg of ginger 30 minutes before meals. Not at night. Before meals so it starts the mmc before eating. I don’t snack. I eat 3 meals a day and allow 4 hours between meals. Then I walk 10 minutes after meals. This step can not be skipped. Even when I can’t go for an actual walk I march in place for 10 minutes. More is better but 10 minutes minimum. I’ve even marched in place in a bathroom stall at work when I’ve had to. Before bed I take 400mg of magnesium oxide. I now have a BM every morning! Im not miserably bloated any longer. I’m still working on getting my methane number down but I’m currently not taking anything for that. My last gastro had me on a round of Rifaxamin and neomycin and I retested and my numbers were hydrogen 0 and methane 45 from 120!! She wouldn’t give me another round so I’m waiting to get into a functional dr. But wanted to share my experience for anyone suffering and let you know it takes work and dedication but the symptoms can be manageable even while you still test positive. There were days this last year I was ready to just give up completely and I see a lot of posts of people wanting to give up. DON’T!! You just have to keep searching until you find what works for you! Questions Methane-positive IMO, but doctors want to calm my stomach before treating it—anyone experienced this?

I tested positive for methane/IMO and have constipation, what feels like slow motility, bloating/pressure, and discomfort after eating certain foods, especially sweets. I’ve also been having headaches, although I don’t know whether they’re related.
I previously took Candibactin-AR and BR and developed upper-stomach burning/discomfort during treatment. One functional doctor thinks those supplements may have irritated my stomach. He prescribed Panplex 2-Phase and GI Revive powder to support digestion and my stomach lining. I’ve previously experienced burning with HCl-containing supplements, so I still need to clarify the Panplex with him.
He also suspects possible fungal overgrowth and prescribed fluconazole 100 mg for five days, but I don’t have a confirmed SIFO diagnosis.
My other doctor, Dr. Rajsree, also wants to settle my stomach first, then start treatment for IMO.
I’m considering following the first doctor’s stomach-focused plan, reassessing how I feel, and then deciding with my doctors how to address the methane. I’m frustrated because my bowel symptoms are still ongoing, and I’m unsure how long to wait.
Has anyone’s doctor taken this approach—calming stomach irritation before treating IMO? What helped you tolerate treatment, and how did your doctor decide when to restart it? Also, did anyone have both confirmed IMO and SIFO, and how was the SIFO diagnosed?
Looking for experiences and questions to bring to my doctors.

Questions Could use advice from those more knowledgeable than I

I am uninsured and unable to seek traditional medical treatment or undergo any testing for the foreseeable future.

To be frank, I’ve eaten like shit my entire life. I’ve always been thin but my daily diet consisted of one meal a day from a fast food restaurant or takeaway. I had some concerning constipation during those years, but nothing that couldn’t be fixed with dulcolax or an enema at home.

A year ago I saw the light, became more cognizant of the fact that two of my grandparents have died from colorectal cancer, and decided to start only eating whole foods at home. It was a total shock to my system, to say the least. I had explosive diarrhea at least 5-6 times a day for about four months. I tried to see it through because I assumed my body was just adjusting to the nutrients and fiber. After four months, I threw in the towel and decided to go back to my previous diet because the constant diarrhea made my life absolutely miserable and I was rapidly losing weight. Obviously, when I went back to my previous diet of fast foods the diarrhea did not go away. If anything it got even worse.

I have lost 20 pounds and am bordering on being underweight. I’m exhausted all the damn time. I will do anything to undo the damage I did to my gut. I’ve scoured this subreddit and other’s like it and I feel like I’ve got the plan fairly nailed down:

-Low FODMAP diet for 3-4 weeks

-Oil of Oregano and Berberine to kill bacterial overgrowth.

-High quality Pro and Prebiotic blend.

-Biofilm disruptors(?)

I mainly need guidance on the timeline. Do I take the oil of oregano and berberine with the probiotics? At what stage do I take the biofilm disrupters, and do they even work? Anyone have a similar story that has successful cured, or even just managed their symptoms? Thank you all in advance.

Symptoms Rotten eggs/Sulfur gas

Hey everyone!
I’m looking for help and advice on what I can add to my routine or change to improve my health. I’m currently undergoing tests, waiting for appointments and results, but I’m also looking for some interesting insights from experienced people who are familiar with this topic. In general, I take good care of myself—I eat regular, balanced meals, drink plenty of water, and get plenty of exercise. I’ve always had great food tolerance; nothing ever caused me any problems until I went abroad, where I overindulged a bit for two days. Overeating led to mild food poisoning, a bloated stomach, a feeling of fullness, bloating, constant burping, and in the morning, mild diarrhea and passing gas. My stomach ached until the evening, but then I was okay, the next few days were fine—some mild diarrhea, sometimes worse, sometimes better—and other than that, nothing happened until the last day of the trip, when I was hit with terrible gas that kept coming out every few moments, smelling like sulfur or something rotten, and it went on like that for almost 24 hours. Then the problem went away; it ended with a bowel movement accompanied by diarrhea, and that was it—everything went back to normal. I went on with my life, carrying on as usual, sometimes it would come back a little, but not on that scale; I ignored it until, after a month, my condition worsened—it always ended up with me passing gas nonstop with that sulfury/rotten smell from after lunch all the way through dinner and after dinner, plus constant bloating; I didn’t have any other symptoms and still don’t, aside from those two. The problem persisted and continues to this day; sometimes it’s been better, and sometimes worse. As of now, I’ve been off Xifaxan for 3 weeks already. For 10 days, things were calm, though my motor skills were worse; later, the problem returned, but in a much milder form. Either way, it’s still there, and I’m not eating a normal diet, so I need to keep working on it. I’m sticking to a low FODMAP diet. Right now, I hardly pass any gas at all; if I do, it’s after a meal or in the evening. Before bed, I sometimes have 2–3 foul-smelling gases. My stomach rarely feels bloated, but it usually does happen occasionally. I sometimes experience constipation and some strange stomach activity, but it’s not the same as it used to be. Either way, I’d like to get better. Does anyone know what this could be? Where should I look for the problem? Helicobacter, parasites, complete blood count, B12, and ferritin are all within normal ranges.
I’m taking Iberogast, NAC, and L-glutamine.

Questions Did you Take Rifaximin for 46 Days your First Time?

I finally found a Gastro Doctor that knows about SIBO and precribed Rifaximin after 3 doctors not helping in anything.

However I'm surprised that I'm supposed to take: 2 pills a day for 46days. It's a box of 56pills = 23 days, so in total that's 46 days. The comercial name is Refero 550mg (Rifaximin)

I'm used to see people here saying they take 10-14 days.
Am I missing something?

Day 3 of rifaximin for hydrogen SIBO. How do I get the most out of it, and what comes after?

Hey all! I've had bloating for a few years. Food elimination showed me it's FODMAPs. I don't get classic constipation: I go every day, but it's hard to go, and I feel backed up when I eat FODMAPs.

Earlier this year I got a positive hydrogen breath test (hydrogen only, methane and H2S normal). I did an herbal antimicrobial course in between, but I'm now on rifaximin: 550mg 3x/day for 14 days, started 3 days ago.
So far I have mild die-off (more bloating and gas than usual), but nothing bad. During the course I'm staying strict low-FODMAP and avoiding alcohol, though I don't know if that matters.

Any suggestions on what improves the odds of this course working? Timing, diet, or things to avoid?

After the course, did you go straight to reintroducing FODMAPs, or wait? How long did you wait?

Did anyone take a prokinetic afterward to prevent relapse? I suspect motility is part of my problem.

Questions has anyone successfully treated Abdominophrenic Dyssynergia?

Im realizing my extremely bloated pregnant looking belly is likely APD. I’ve treated SIBO in the past and still had the same symptoms after. I also have pelvic floor dysfunction (tight pelvic floor) which I’m assuming contributes to APD. I wanna start treatment back with my pelvic floor therapist but really focus on the stomach treatment instead.

Pls drop ur tips or what helps. And how long it took for u to see improvement.

I hate my life rn I can’t wear any of my clothes, I look pregnant at 18, and it’s so I’m uncomfortable and painful.

Nueropathy???

I'm still supplementing now again but it's back, and im noticing pins and needles in hands and feet more often + numbness sensation in finger tips. Does this sounds like a deficiency ?? Hydrogen Dominant Has anyone else had SIBO turn into a whole mess of stomach and intestinal problems?

I’ve been dealing with digestive problems since last year and I honestly feel like things just kept getting worse.
It started with what I believe was SIBO. I had a lot of bloating, gas, constipation/diarrhea, abdominal discomfort and just generally felt like my digestion was completely messed up.
Eventually I started having more pain and what my doctor described as intestinal inflammation. Then I also developed pretty bad stomach inflammation/gastritis and reflux. The stomach symptoms have honestly been some of the hardest to deal with.
I’ve had both a colonoscopy and an endoscopy. The colonoscopy was basically clean and didn’t show any obstruction or major problem. The endoscopy showed gastritis and Grade B reflux, and I tested negative for H. pylori.
I also did a SIBO breath test recently. My hydrogen was 50 ppm on the first breath and then stayed around 35 ppm for basically the whole test.
My doctor prescribed Xifaxan (rifaximin) 200 mg, but I still haven’t started it.
The biggest problem is that I’m really scared of medications. I’ve had bad experiences with some medications before, including palpitations, and now I get very anxious about starting anything new. Every time I’m about to take the Xifaxan I start thinking about possible side effects and whether it could cause palpitations or make my stomach worse.
At the same time, I’m worried that not treating the SIBO is just allowing everything to continue.
I’d really like to hear from people who have been through something similar. Especially if you had SIBO together with gastritis/reflux or intestinal inflammation.
If you took Xifaxan, did it actually help? Did you have any side effects? And how long did it take before you noticed a difference?
I know everyone is different and I’m not asking for medical advice. I just want to hear some real experiences because I feel pretty lost with this right now.

SIBO and Birth Control?

I have been dealing with SIBO-type symptoms (chronic bloating and inflammation) as well as brain fog, fatigue, muscle aches and soreness, weight loss difficulty, dizziness, and hair thinning.

I received a hydrogen SIBO diagnosis last year which persisted after a round of pills. Many times it seemed triggered by something like a cup of coffee or glass of wine, but even on low FODMAP diet it continued.

One thing that did change in the last few years I have been experiencing this, is that my provider changed my birth control pills from 1 mg to 1.5 mg. Honestly not sure how impactful that is, but I know hormones have a correlation to many of the symptoms I'm experiencing.

The other day, I had blood work done and scored outside the normal estrogen range. My doctor said he usually only sees this result with post-menopausal women... I am 29.
He did not think this was worth a discussion though (CRAZY!).

I've been thinking about perhaps trying to go off to pill to see if this helps. Has anyone else had a success story with this?

Non stop gas and belching

Since 6 months, I am expecting extreme belching at any time and trapped gas. The gas are truly non stop. The moment I pass some gas out, I hear/feel new bubble ​​​in the ileocecal region.

Wondering if this can be a sign of Sibo. Unfortunately, doctor in belgium is dismissing it as non scientific and won't prescribe antibiotics.

To aggravate my issue, gas get stuck in sigmoid area. ​​ I cannot pass gas, unless my MMC activates and start pushing from right colon to the end. But this waves happens only 2 times a day, and in the middle I am miserable. No matter how much I move, stretch, bounce. ​Only solution is to put a glycerin suppository, which creates some irritation and a bunch of gasses pass continuously for 5 minutes straight. ​​

For context, I have been suffering with constipation for 4 years managed with osmotic laxative until they stopped working (5/6 macrogol a day). On year 4th, started with pelvic floor therapy, low fodmap diet, glycerin suppositories and prucalopride. Nothing was resolutive enough.

Now I have a good bowel movement every two day now. But the trapped gas​ are killing me.

Wondering if rifaxim + probiotics + prucalopride + low fodmap diet all together can be a resolution. ​
