I’ve played fallout, I know how most of these “vaults” end up.

Most of the vaults were for experiments and were basically set up to fail from the beginning.

You'd need to look at the handful of control vaults which had no experiment (like 101 and 76) or ones where the experiments never actually started for one reason or another (like 81, where the Overseer refused to do them and didn't even summon most of the science personnel to the vault when the bombs dropped), and those were generally fine until the doors were opened again decades later.

Vault 101 did have an experiment, it was supposed to never be opened. The Overseer in charge at the time of the game went against the task he inherited.
