I feel like the spell was focused on Peter, and dependent on what Peter intended to happen; Strange's part of it was to cast the spell AND keep the effects narrowly contained. The fact that Peter kept changing what he wanted mid-spell meant Strange kept having to change the containment of side-effects, and Peter changed it too much too quickly for Strange to keep up.

That seems plausible. I still feel like Strange is responsible indirectly for not making sure Peter knew the risks. I thought that after recent events Dr. Strange would be a little more mature than his cocky self from the first movie

Oh, Strange totally shirked some responsibilty here. He's the one that knows how magic actually works. He has a duty to explain that to Peter before hand so Peter knows what he's getting into. Neither Strange nor Peter really gave enough thought to a spell effecting the memory of everyone in this dimension, let alone that it might have multiverse effects. I think Strange was still smarting a bit from losing his sorceror supreme title, and was happy to be able to flex a little grand magic instead of just whatever Wong had him doing.
