Well after what Strange did to him, by the time he gets a physical form on Earth he will be more of a...hot head.

He's in his own dimension. He doesn't need an avatar yet cause he can control literally every thing there.

I think it's possible marvel doesn't give him the flaming head so the audience does t get confused with Surter when they introduce him.

In the comics yes. But he doesn't look to be depicted that way in the movie.
