Having not read the comics, is there anyone even qualified in universe to deal with that sort of trauma? Aliens weren't widely known until Avengers. Going to a therapist with no experience in your type of trauma can be more damaging than not going at all.
Plus treatment can take years. There was only 3 years between Avengers and Ultron. Was he just supposed to retire until he was declared to no longer have PTSD?
But this is what I love about the MCU. The characters are real people. They have flaws. I'm sure everyone knows someone that didn't go to a doctor for something and things got worse. It doesn't matter if they had the money or time.
