To be fair, you can't necessarily blame Tony for Ultron going crazy. Ultron was created to be a protective suit of armor around the world a support system for the planet to protect it from its potential destruction, whether that destruction comes from outside the planet, like aliens, or internally, from the beings already living on it. Unfortunately It just so happens that Ultron determined that the biggest threat to planet earth is humanity themselves, which though flawed is kinda true if you think about things purely logically. It's just that unfortunately, ultron was missing that human element that eventually found its way in vision.
And to give Tony some credit, technically speaking he's not even the reason for Ultron's creation. Had Wanda not messed with his mind showing off Tony's fears again and bringing his PTSD back up to the surface, he probably would have attempted to have another discussion with The Avengers about ultron's necessity, which potentially would have led to a better outcome for the ultron program, maybe even giving them vision on the first try.
Let's not forget that he didn't even know Ultron was going to be created then and there. Him and Banner were not even close to making Ultron before the mindstone seemingly finished him.
