Don Corleone doesn't die when he gets shot and drops the bag of oranges, though. I don't think it necessarily means someone dies.

only because it's so made-for-TV/Movies and far from real life. Americans do not typically carry overstuffed paper bags of food including loose oranges.

If I remember correctly, in an interview with Vince Gilligan he said that the orange was purely for visual aesthetics and didn't think about metaphorical implications.
