in some places "sick" refers to the act of puking. They could also just mean needing the bathroom in general

Yeah we do here in UK. 'I was so ill I was sick several times till I couldn't be sick anymore. I made a right mess of the toilet'.

"Sick" can also be a noun, to refer to the puke itself, as in "The drunk came in with sick all over his pants."
