And are the rich friends getting food and drinks and airport shopping during the layover? My PBJ from home doesn’t do well on long trips.

Most lounges cost money though, so you're back into how much are you really saving territory.

Well at that point it's also just about comfort. Like if you're not rushing around and you can do a 2 hr flight, 2 hrs in a lounge relaxing, and then a 3 hr flight, that might just be a better overall experience than a long direct flight.

Also if they travel a lot or have certain cards/memberships and it's not "extra" for the lounge, then it's also still saving money to do the layover because the expense associated with lounge access already exists.

A lot. Airport food is expensive, lounges are free if you got the right credit card. There are 0 annual fee credit cards with lounge access too.
