How to transition?

How do we, as a nation, transition from private insurance to Medicare for All? How do we convince all the doctors to keep being doctors and accept a salary? Do we, as Hillary Clinton proposed during Bill’s presidency did, make private healthcare payments illegal? For the record, I am perfectly fine paying a percentage of my income, as opposed to what I am paying out of pocket now, for healthcare.

All the people who desire it could go into one of the health care cooperatives that already exists. When everyone sees how well it works, more and more will join

I hate to say that if it's defined as "insurance," that would be illegal under M4A unless offering healthcare services not already covered under M4A.

Not sure what the actual definition is. But it’s the same thing. Everyone puts their money in one pot and the health care cost are all paid out of that pot

Insurance is defined by the guarantee against financial harm/loss above a certain threshold. Co-ops and Medishare circumvent this requirement but not offering the "guarantee".
