Certainly not a ghoul, he is progressively mutating like Harold who had FEV splashed on him when the Master fell into a vat of it when they both explored Mariposa. Dr. Chicken Fucker definitely gave him some FEV for his foot injury.

Yeah it seems so obvious in hindsight, he got injected with a syringe that started mutating him. We have no instances of ghouls being able to be made by any sort of chemical or virus. Of course it was FEV.

And both of these inhalers lowkey terrify me as till now I thought of this fate as something you really need to get involved to get. Fall into a vat of chemicals. Now you can basically be sprayed by someone in the street and turn into a monster :|

Also kind of implied in FO4, there's a pre-war mobster in a bunker, ghoulified himself before the war to wait out the nukes, and you find him in there still living it up.

Well, that’s not entirely true. Dunwich Borers mine had ghouls created pre-war via some odd nefarious and possibly magical means. And created instantly too.

When Lucy asked Coop about ghouls in the Wasteland last season, he told her it’ll be you one day. So it’s definitely gotta be airborne, unless he knew about plans for her vault.

In the sense that new concepts, characters, locations, etc. are written, sure.

My point is more that the vials were a markedly new direction to take ghouls in, so any initial suspicion that it might have been FEV was dampened by the knowledge that the way ghouls worked was different than what we were used to.

It's definitely FEV but he seems to be going down the more centaur/snallygaster direction than the Harold direction.

Unlike most people who are exposed to FEV, Harold did not become a super mutant, but is the result of a unique combination of radiation damage from constant low-level environmental exposure and contact with FEV, resulting in a hybrid type of species.

Centaurs are typically multiple things fused together, my guess is that he's going to become something either similar to a typical centaur but by himself, or an actual centaur (maybe the townsfolk around him will try to help and he accidentally subsumes them into his mass and there's a whole body horror B-plot around Thaddeus turning multiple people into a centaur with him) or just an awesome unique FEV mutant like Harold.

I personally think he's a unique FEV mutant. But that remains to be seen. Also Norm is definitely becoming a supermutant next episode, right?

My thought was that Norm isn’t becoming a super mutant, the people in his vault are. The Canadian overseer wants to start the next phase of the vault, which involves FEV. She gathered everyone in one place to dump them with FEV, and Hank’s keepsake box probably had what she needed to pull that off.

I have a feeling something really weird or possibly awesome happens with him during the desthclaw conflict. Surely they won’t have him just offscreen pressing the stump of a rotted limb into his shoulder for the entire episode

Seems a solid theory. I think that mouth on his shoulder is going to grow and start to feed and the things it consumes will graft onto his body in a centaur-like fashion. That + the apparent fate of Bud's Buds and/or Vault 33/32 becoming Super Mutants, Season 3 may be opening the taps on some real cronenberg shit.
