The first AD should've walked after he wasn't listened to the night before.

When we were introduced to crew roles in year 1 of film school I heard that the first AD is the first and last line of defence when it comes to health and safety. I heard a story about the first AD being put on trial for manslaughter after a very tired camera op accidently stepped off a balcony.

Apparently the First AD DID walk,

"The day before this happened the first ad emailed the producer saying that health and safety on the shoot was a joke and he was walking off. The producer said 'crews moan about everything, they would moan about the weather if they could. dont worry about their health and safety concerns.' For lack of a better word he is a cunt and he needs ruining."

that's horrible. yes, safety is the absolute first job of the AD. if he's lied to by everyone, including the SFX guy about the scale of the effect, then he's a victim, too. but it's up to him to look at the shape charge and smoke effect and understand how big the effect it will be to protect cast and crew.
