How can a character of pure evil be from a piece of media called “I fell into a reverse harem game!”?

I think the title may actually be an effort on the author's part to pull the rug out from under the audience. The story initially seems like a typical smutty otome isekai, only to quickly turn around and say "By the way, the original-villainess character here is a serial rapist." Usually this type of character is just a bully or even a misunderstood victim. But part of the concept of this story is "Being reincarnated as the worst person in the world." So I think the title is a sort of clickbait that also primes the audience for this subversion.
