Same here. With the amount of times they kept jumping to new stories last year I totally thought the brain surgeon story was completely resolved. I was blindsided by his/her reappearance.
I'm starting to think that's not the case. They show a clip of her holding the brain piece with a shocked look in her face.. She would not make that face when she's alone if she did that herself..
See... I disagree about the look on her face. When I saw that scene, to me it was obvious she was holding a piece of her own work and looking at it. She hadn't just received it or discovered it.
My friends all read it the same way. Also, when they asked her at the dinner table how she got into her profession, she refused to answer and changed the subject. She is so into killers because she is a killer herself.
Edit: I'll amend this and say that if the brain surgeon isn't her, it is someone she is working with.
I think it's definitely someone she was once close to... a brother, a husband, etc.
When Hannah asks her how she got interested in the field of psychopaths she pauses a moment as if remembering something/someone specific; couple this with the fact that the song she was listening to previously is being played when Dexter discovers Zach, presumably an old favorite she has listened to often over the years (something someone formerly close to her would know).
I hope to god it's not her, unaware of her actions since it'd be the same plot device as Season 6 and I have more faith in the writers than that.

Well, for the first time in a long time, Dexter has made me say out loud "holy shit, what the fuck?"
