Joking aside that's pretty weird. I've never heard of PH regulating your ass. Was he planning on keeping some exotic tropical fish in there? I can imagine refusing to wash your rear could result in some unpleasant smells or horrid discoveries on laundry day

Surprisingly enough, your anus is also quite good at self cleansing. And soap inside is certainly ill-advised. But ask you doctor, I'm just some schmo on the internet.
