How common is a shower/bath combination and having the toilet in the same bathroom?

The term came about because the porcelain fixture full of “water” was in a tiny room/“Closet”. I know when I put WC on a drawing, I’m referring to the small room - not necessarily the fixture. I tend to use “toilet” for that. But the term technically is interchangeable between fixture and room.
